MY FIRST VISIT
Contact our team to schedule your initial appointment. Once scheduled, you will receive a welcome Email and invitation to our Patient Portal where you will complete your new patient forms online.
Please complete your forms through Patient Portal at least 2 days prior to your appointment if possible. Also send a copy of your driver's license and dental insurance card in advance so that our insurance team can perform a complimentary benefits assessment for your policy prior to your arrival. Plan to arrive 15 minutes early for your scheduled appointment.
Upon arrival, we will create your new patient chart and acquaint you with your new dental home and dental family. We want to ensure that you have a great dental visit and feel at home at Dental Wellness Centers!

"Network Provider" For Most Insurance Plans
We welcome dental insurance! We will be happy to answer all of your questions and contact your insurance provider to perform a complimentary benefits check for you during your initial visit to our practice. As a courtesy to you, we are happy to bill your insurance company for you. Most insurance plans offer both "in network" and "out of network" benefits that can be utilized for services at our dental practice locations.
Complimentary Benefits Verification
Dental Wellness Centers of GA providers offer the opportunity to file claims on our patient's behalf with most dental insurance carriers and their associated plans. Our team will conduct a complimentary benefits verification prior to your dental appointment when your plan information is provided to our team a minimum of 48 hours prior to the appointment date/time. Please note that it is the patient's responsibility to provide accurate information to the team for verification purposes and our team can only provide estimated out of pocket expenses based on the information that your insurance plan provides to the practice team. We do require a copy of the dental insurance card and drivers license on file for all patients who desire assignment of benefits from a dental insurance plan to our dental practice.

Dental Wellness Center offers an in house Preferred Dental Program: (DWCPDP.)
We consider our patient's needs and finances a priority. In light of the economy, our practice has adopted a new concept: an alternative to the "norm" of dental insurance.
Affordable Dental Care
The DWCPDP is an affordable way for our patients to obtain preventive dental care and discounts on dental treatment that they may otherwise not be able to afford. Patients know upfront exactly what their dental care costs are.
Treatment Discounts
Our program covers Preventive Care at 100% for healthy mouth condition with 20% discount on all Periodontal Therapy.
The program offers a 20% discount on most in office treatment, restorative (composite restorations, aesthetic veneers, crown and bridge) procedures, sealants, dentures, endodontics, oral surgery, prosthodontics, dentures, periodontal therapy, whitening procedures and other miscellaneous dental services.
The program offers a 10% discount on orthodontic treatment, implant treatment plans and sleep apnea therapy.
